Locating the pain
The patellar tendon may be painful below the kneecap; the quadriceps tendon above it. This pain is often related to repetitive stress, particularly jumping. Tendinopathy is not necessarily a simple inflammation.
Crow's feet and iliotibial band syndrome
Pes anserinus can be painful on the inner side, below the knee, sometimes with associated bursitis. Iliotibial band syndrome affects the iliotibial band on the outer side, particularly in runners. These conditions are not all identical tendon injuries.
The Assessment
The examination locates the painful structure and assesses the effects of sports activities. An ultrasound or MRI may be useful if there is doubt or an unusual progression.
Treatments and follow-up
Load adjustment and progressive rehabilitation form the basis of treatment. Strengthening and return to sport are individualized. Persistent worsening necessitates a reassessment of the program rather than increasing the number of painful exercises.
