Images and reports of X-rays, CT scans, or MRIs already performed; letters and surgical reports if available.

Current prescriptions and list of treatments already tried. Also note any known allergies.

The activities that bother you, the progression of the pain, and what you expect from the consultation. For a second opinion, bring the treatment proposal you received.

Your administrative documents
Health insurance card, supplemental health insurance certificate, and referral letter, if applicable.
If a procedure is recommended, the preparation, hospitalization, and follow-up procedures will be explained to you during your consultation.
For postoperative medical questions, follow the instructions and use the contact information provided upon your discharge. The secretary can forward your request to the surgeon.
