Recognize the Injury
These tendons transmit the force of the quadriceps to straighten the knee. A tear can cause sudden pain, swelling, and an inability to raise the leg straight. Prompt evaluation is necessary.
The Assessment
The examination tests active extension. Radiographs assess, in particular, the position of the patella; an ultrasound or MRI can determine the extent of the tear.
Treatments
Some partial tears with preserved extension may be treated with a brace and rehabilitation. Complete tears generally require early surgical repair. A long-standing injury may require more complex reconstruction.
Protecting the repair
Orthotics, weight-bearing, and flexion exercises are prescribed according to the repair. Strengthening progresses without prematurely putting the tendon under tension. Stiffness, residual weakness, and re-rupture are possible.
